Players: At registration please bring the following: Medical Release Form. THE ABOVE FORMS ARE MANDATORY FOR ANY PLAYER WHO PARTICIPATES IN CAMP. IF A PLAYER DOES NOT HAVE THE FORMS OR IF THEY ARE NOT FILLED OUT CORRECTLY, THE PLAYER CANNOT PARTICIPATE IN CAMP. If staying in hotel, please make your own arrangements. Although the camp is held on the campus of Murray State, it is not owned or operated by the university.
